Please note that upon making your initial appointment,  we will provide you access to your own Patient Portal in our electronic health record system (PrognoCIS).  We highly encourage you to use the Patient Portal to avoid having to fill out paperwork at the time of your appointment.  Your use of the portal is beneficial to your healthcare and to our practice.

(*Patients should know that the federal government and some insurance companies require doctors to report the level of use of the electronic health record (EHR) by patients as a "quality-of-care" measure.  They rate us on how well we engage our patients in the EHR.)
